In addition to private-pay Homemaker and Companion Services, CAP REVIVAL CARE LLC is an approved provider for the Florida Agency for Persons with Disabilities (APD) and offers services through the Home and Community-Based Services (HCBS) Medicaid Waiver Program and the Developmental Disabilities (DD) Medicaid Waiver Program in accordance with applicable Florida statutes, APD requirements, and regulatory guidelines. We are also a participating provider with Florida Community Care (FCC) and Aetna Long Term Care (LTC) Medicaid plans and programs, which support individuals with intellectual and developmental disabilities through managed care and community-based service delivery models.

Rates and reimbursement for HCBS and DD Medicaid Waiver services are established and regulated by the federal Centers for Medicare & Medicaid Services (CMS) and the State of Florida through the Agency for Health Care Administration (AHCA) and APD, ensuring access to quality, cost-effective community-based care.
